SUMMARY:

HIV infection is a global health challenge, with the WHO setting a goal to
eradicate AIDS by 2030. In 2023, approximately 39.9 million people were living
with HIV. In Brazil, renewed health policies aim to reduce virus transmission and
increase treatment adherence. Primary Health Care (PHC) plays a crucial role in
this process by promoting health education and ensuring access to treatment.
However, challenges such as high staff turnover and work overload persist. This
exploratory, descriptive, and qualitative study was conducted in the metropolitan
area of Macapá, Amapá. In the first stage, a State-of-the-Art literature review
analyzed the decentralization of care for people living with HIV in Brazil. In the
second stage, interviews were conducted with 36 healthcare professionals
providing care to this population in four Primary Health Units (PHUs). Data
collection took place between March and June 2024, with interviews recorded,
transcribed, and analyzed using ATLAS.ti software, resulting in three discussion
categories. Findings revealed that, despite the implementation of rapid tests and
occasional training sessions, care decentralization still faces obstacles such as
social stigma, work overload in PHUs, a shortage of trained professionals,
resource scarcity, discontinuity in post-diagnosis follow-up, and service dropout.
The emerging categories were: How the decentralization of care for people living
with HIV is presented to professionals in Amapá; Main challenges of the
decentralization process; and Strategies for consolidating decentralized care.
High staff turnover in Macapá’s PHUs was a limitation of the study. Therefore, it
is essential to implement continuous training, provide multidisciplinary support,
and develop public policies that strengthen health units. These measures are
fundamental to ensuring comprehensive care and reinforcing the SUS in the
region.
